Assumption and Acknowledgement of Risk and Release of Liability Agreement
In applying to enter my son(s), daughter(s), or relative(s) in Aloha Surf Camp's surfing, watersports, and teen camp program, I promise to inspect the camp and surfing site, and assure myself that the area is safe for surfing, trampolines, tubing, wave runners, paddleboarding, snorkeling, dodgeball, and other beach sports.
I further agree that I will not participate nor allow the below named minor to participate unless I am satisfied that the area and conditions are safe for all camp activities.
I understand and will comply with all of Aloha Surf Camp's rules and regulations, and do agree that I voluntarily assume all risk arising out of all watersports, surfing activities, and conditions relating to camp.
In consideration of your acceptance of this entry, I, intending to be legally bound, hereby, for myself, son(s), daughter(s), relative(s), heirs, executors and administrators, hold harmless and release and forever discharge the Aloha Surf Camp / Surf School, Paul Vargas, Palm Beach County Parks and Recreation, Palm Beach County, Town of Jupiter & Juno Beach, Aloha Surf Camp's sponsors and all their employees, agents, subcontractors, independent contractors, servants, including but not limited to my own negligence and the negligence of the above-named parties, arising out of my or the herein named minor's presence or participation in the above-described event and as described in the registration form, which is incorporated herein in full.
I attest that I, my son(s), daughter(s), or relative(s) is physically fit and is sufficiently trained for surfing, and do hereby grant the above-named parties such releases as described herein.
I hereby represent and warrant that I have authority to sign this document on behalf of the Student.
